In order to continue serving our communities we kindly ask for the help of parents in collecting and bringing in essential documents. Failure to provide documents could jeopardize our ability to serve your child into the future.
When:
Nov 27th to Dec 1st All Day On Campus
Where:
At Rio and Arecibo Campus Locations in Administration Office
What is needed:
All Parents need to bring the following documents:
Documents all Parents need to supply
- Duly signed Agreement Package
- Two (2) 2x2 photographs of the student
- Birth Certificate (we do not keep the document)
- Social Security Card (we do not keep the document)
- Dental Certificate (only for 6th and 8th-grade students)
- Vaccination Certificate (Green Paper)
- Transcript of Credits from the previous school (original and in a sealed envelope)
- Proof of address (electricity, water, or phone bill)
- Results of the META PR tests (or other standardized tests if coming from a private school)
- Enrollment Movement document If registered in Special Education, please provide us with the IEP (Individualized Education Plan)
- Legal custody/guardian document (if applicable) Others:
- Copy of the medical plan card
- Copy of parents/legal guardians' identification
- Documents of the reasonable accommodation plan (if applicable)
